What information do we collect from our customers? Elevated Academy collects the following information when you sign up for a trial class or membership: 

• Parent’s/Student’s Name 
• Child’s Name (if a minor child is the student) 
• Phone Number 
• Email Address 

If you purchase a membership, we also collect the following information:  

• Billing address 
• Credit card or bank account information (for tuiƟon payments) 
• Your birthdate and your child’s birthdate. 

∙ How do we collect it? We collect this information either in-person when you purchase a membership or through the Wodify system when you sign up for a trial class or membership online. 

∙ Why do we need this information and how do we use it? We collect billing addresses to billing for tuition; we collect phone and email to provide customer service and billing/contract noƟces; we collect birthdates to track progress through our training programs according to age.

∙ Do we share your information with anyone?  We do not share our customers’ information with anyone. 

∙ How can customers opt out of communicaƟons with us? You can text STOP, UNSUBSCRIBE,  QUIT; or by unsubscribing from emails in the Wodify app.
